At Lyft Community Live Well at Home, we welcome feedback and take concerns seriously. We believe good care includes openness, reflection and a willingness to keep improving.
If something does not feel right, we want to hear about it. Raising a concern will never affect a person’s right to receive respectful support.

How to raise a concern
If you have feedback, a concern or a complaint, you can contact our team directly. We will listen carefully, respond respectfully and work towards a fair outcome.
If you would prefer to speak with an external body, we can also provide information about independent advocacy and aged care complaints pathways.
